Insurance
Student Group Insurance:All NTUST students have to participate in the student insurance program and pay for the insurance fee.
■The insurance coverage includes:

Illness : claims are only available for hospitalization.

Accidental injury : outpatient clinic or hospitalization, both can apply for insurance claims.

■When applying for an insurance claim, you need to submit the following documents:
1. Application form
You may pick up the form at counter No.20, the Office of Student Affairs or download on the website:

2. Original copy of medical diagnosis certificate
3. Hospital receipts (original or certified true copy)
4. Certificate of tuition payment
▶The certificate should be for the semester in which the incident occurred and medical treatment

5. Photocopy of your own bank passbook and ARC(front and back)
6. Others:
▶Fractures: Additional "Hospital X-ray Disc" is required
▶If you did file a report: the report form from the police is recommend
■Notes:
- The insurance claim must be applied for within 2 years after the accident or illness occurred.
- If the accident or illness occurred overseas, English version of local medical diagnosis certificates and hospital receipts are required.
- If you have further question regarding the documents mentioned above, please email: jamiewu@mail.ntust.edu.tw, or come to counter No.20 in the Office of Student Affairs during business hours (Mon.-Fri. 9:00-12:00, 13:20-17:00).
- After applying for an insurance claim, it usually takes about 5 weeks for the insurance company to review your case, and if you have any concerns during the process, please contact the insurance company.
